Home Health Services
The hospice care team at Summit Hospice, made up of skilled professionals and trained volunteers, provides expert medical care to keep patients comfortable and able to best enjoy time with loved ones. The hospice team answers questions, offers advice on what to expect, and helps families with the duties of being a caregiver. The team also provides emotional and spiritual support for the entire family.
Home medical equipment and related medication
Equipment such as hospital bed, wheelchair, oxygen, etc. Medical supplies such as briefs, wipes and all medication related to the terminal diagnosis.
Pain and symptom management
All medications are covered for symptom control and pain relief according to the hospice formulary. Nurses will assess each visit and collaborate with patient, family, and physician to adjust as necessary.
Spiritual and emotional support
Chaplain, social workers and nurses are available to provide spiritual and emotional support
Comprehensive bereavement program
Bereavement services provide ongoing grief and loss support for the bereaved over a period of 13 months.
Hospice Medical Director
Provides medical direction for the patient’s plan of care
Regular home care visits by registered nurses and licensed vocational nurses to monitor the patient’s condition and to provide appropriate care and maintain patient comfort
Provide counseling services, referrals to community resources, and assist with advanced directives.
Home health aide visits provided to assist with personal needs such as bath/shower, etc
Provide spiritual support for patient, families and caregivers according to their desires and beliefs. Chaplains can provide assistance by connecting people with clergy of their own faith group or other professionals
